Pulmonary artery catheterization and wedge pressure measurement in the general intensive therapy unit.

چکیده

Central venous pressure measurement may be misleading in selective left heart failure and in patients with high pulmonary vascular resistance. More direct information about pulmonary venous pressure and left heart filling pressures may be obtained by catheterization of the pulmonary artery with flow-directed balloon catheters. The use of such catheters, without radiographic control, by those inexperienced in orthodox cardiac catheterization is described. The catheter may be used also in the measurement of cardiac output and in the treatment of a massive pulmonary embolism.

Detection of left ventricular failure in patients with adult respiratory distress syndrome.

Fourteen patients with adult respiratory distress syndrome were admitted to the medical intensive care unit and received bedside heart catheterization with the use of a balloon-tipped, flow directed catheter. Four of the 14 (29 percent) were found to have left ventricular failure (LVF), defined as a pulmonary artery wedge pressure greater than 12 mm Hg. Analysis of the standard clinical and lab...

Correlation of blood volume values and pulmonary artery catheter measurements.

OBJECTIVE To correlate the values generated by direct total blood volume measurement with pulmonary artery catheter parameters and commonly used laboratory values in the management of critically ill patients. METHODS This study was carried out at the Lutheran Medical Center, Brooklyn, New York, United States of America, during the period 1998-1999. We prospectively correlated the total blood ...

Prospective study comparing different echocardiographic measurements of pulmonary capillary wedge pressure in patients with organic heart disease other than mitral stenosis.

In 25 patients with cardiac disease, but free of left ventricular inflow obstruction, the electrocardiogram and M-mode echocardiogram of the aortic root, left atrium and both the mitral and the aortic valves were obtained simultaneously with the pulmonary artery wedge pressure (PAWP) during right heart catheterization. The echocardiographic measurements of the left atrial size, PR-AC interval, ...

Segmental pulmonary angiography in the critically-ill patient using a flow-directed catheter.

Selective pulmonary angiography was performed using a flow-directed, balloon-tipped catheter in 20 consecutive intensive care unit patients requiring heart catheterizations on the right side for hemodynamic monitoring. No morbidity was encountered from this procedure. Adequate quality balloon-occlusion angiographs were obtained with a portable chest roentgenogram in 17 (85 percent) but appeared...

An evaluation of wedge pressures in dogs under conditions of normal and elevated pulmonary vascular pressures.

Fifty-six combined right and left heart catheterizations in normal dogs and clogs with experimental mitral stenosis failed to show a significant positive correlation between pulmonary artery wedge pressure and pulmonary vein pressure when the latter was below approximately 17 mm. Hg. Evidence is presented which suggests that the wedged catheter outlets are totally occluded in many instances in ...
